To arrange your online dementia assessment, please select a suitable date and time, then follow the instructions to complete your payment.
We recommend providing a recent brain MRI result and blood test result before your appointment, where available. If you do not already have these, they should be provided within 10 days after your assessment.
We recommend attending with a carer, relative or family member who knows you well.

To help your clinician make the most accurate assessment possible, we recommend that you provide a recent brain MRI result and blood test result before your appointment where available.
We understand that not every patient will already have these investigations in place. If you do not have them before your assessment, your clinician will explain what is needed and these results should be provided within 10 days after your appointment at the latest.
For online dementia assessments, we also strongly recommend that you attend with a carer, relative or family member who knows you well. Their input can be very helpful in building a clear picture of any changes in memory, thinking, communication or day-to-day functioning.

You’ll receive links to pre-assessment forms which need to be completed at least 24 hours before your appointment.
You will be sent PDF cognitive tests by email. These should be printed and kept ready for use during the consultation. Please do not read or complete them in advance of your appointment.
Your online dementia assessment lasts 90 minutes. You may find it helpful to be in a quiet room with a good internet connection. We strongly recommend that a carer, relative or family member joins you for the appointment.
Your clinician will explain the findings, answer your questions, and outline the next steps. If MRI or blood test results have not already been provided, these should be submitted within 10 days after your assessment.
